Preparing For Your Visit
We ask that you please arrive 30 minutes prior to your scheduled appointment. Our patient registration process takes place after your arrival at our office.
Bring your current health insurance card to ensure accurate and appropriate billing of your healthcare services.
Bring your current photo ID.
We also ask that you bring any medications you are currently taking – prescription medications, supplements, and over-the-counter drugs – in their original containers.
Insurance Coverage
We accept Medicare and most major PPO and HMO insurance plans. As a courtesy, we will file claims with your insurance company. If you have questions regarding your insurance coverage, please call us at (423) 714-0714.
If your insurance requires a co-pay, we will request payment at the time of your visit. Be sure to contact our office if there are changes to your insurance coverage.
